to NT$20,000, we¡¦ll notice you once every 10 days.
- We accept the medical charge by cash only.¡]ATM is

For certain special medical
materials aren¡¦t covered by the National Health Insurance
Bureau, we¡¦ll only charge it with either the patient or the
assigned legal authorized representative person by the
patient to sign the agreement.

- Ward Rules
For reasonable excuses leaving, please check at the nurse
station to make an absent. Nurse would confirm with the
doctor and give a permission. Before leave, please do give
return time and contact phone number; also return on time.
Please keep any valuables or any belongings with you.

- Cell Phone Policy
For not intercept any monitors, ventilators and
sophisticated equipments, please do not use any cell at
cardiac and critical care units.
- Smoking Policy
For your own and patients¡¦ health, please do not smoke at
anywhere in KMHK.

- For any certification applying, we need you to prepare
the following copies¡G

- Guide to verify your eligibility of severity impairment¡G
Through
examination by the attending physician.
Take
a the application form from the Patients¡¦ Service
Department.
Must
signed by the specialty physician.
After
given an embossed stamp by the Patients¡¦ Service Department
then apply to the National Health Insurance.

- Procedure of applying Disability Card¡G
After
verifying your qualification, the physician would give you a
medical certificate.
Please
prepared four 1¡¦inch photos and the applicant ID card.
- Application for foreign laborer¡G
Who
may get the form from Patients¡¦ Service Department or ask
for the agency.
Please
bring own ID. and the physician will verify the applicant¡¦s
qualification.
