For cooperating with the Regulations for COVID-19 Inspection of Taiwan Central Epidemic Command Center (CECC).
KMUH has opened an Outpatient Clinic for COVID-19 PCR testing request by Own Expense.

1. 因工作因素須檢附檢驗證明之民眾。
2. 出國求學須檢附檢驗證明之民眾。
3. 外國或中國大陸、香港、澳門人士出境。
4. 相關出境適用對象之眷屬。
5. 經嚴重特殊傳染性肺炎中央流行疫情指揮中心同意之對象。
6. 因其他因素須檢驗之民眾。

1. Work requirement (include Short-term business traveler).
2. To study abroad.
3. Foreign national, Resident from Mainland China, Hong Kong or Macao departing from Taiwan.
4. Family member of people going abroad due to the above reasons.
5. Approved by the COVID-19 Central Epidemic Command Center (CECC).
6. Other special reasons.

※ For people under home isolation/quarantine who needs to go outside for compassionate reasons should get the permissions from the local Health Bureau.

1. 因工作因素須檢附檢驗證明之民眾：工作證明文件 (例如職員證、工作簽證、公司要求檢驗通知、出差通知書、公務護照、外交護照或相關公務函等證明文件)。
2. 出國求學須檢附檢驗證明之民眾：就學證明文件 (例如學生證、學生簽證、入學通知書)。
3. 外國或中國大陸、香港、澳門人士出境查驗護照或入臺許可證。
4. 相關出境適用對象之眷屬：身分證及相關出境適用對象之關係證明文件 (如戶口名簿、戶籍謄本)；適用對象之工作、就學證明等文件。
5. 其他經嚴重特殊傳染性肺炎中央流行疫情指揮中心同意之函文。
6. 申請原因相關文件。

3. To study aboard: Certificate of enrollment.
4. Foreign passport or the Entry Permit for Taiwan.

6. Approved document by the COVID-19 Central Epidemic Command Center (CECC).
