單位介紹

  • 社區整合型服務中心(A級單位)
  • 主要服務項目：與長照個案(案家屬)討論及訂制照顧服務計畫、長照服務連結、追蹤服務品質、提供申訴管道。

一、服務對象

1. 65歲以上失能長者

2. 55歲以上失能原住民

3. 50歲以上失智症患者(經醫師鑑定)

4.失能身心障礙者

須經長照專業人員評估為長期照顧等級失能程度第2級(含)以上者，才可使用長照服務。
