Important Notes for Outpatient Clinic	Only Morning Clinic available for Saturdays Please bring your National ID card, National IC health insurance card and any related supporting documents (disability card, catastrophic illness card) First visit: (1) Please go to the Physiological Station at service desk and fill out the “Outpatient self-Health Assessment Form” to take Physical assessment. (2)If an appointment is made through the automatic voice registered system, please proceed to the service counter first to present your documents (3)If an appointment is made through on-line or in-person, proceed directly to the clinic directly If an appointment is made through phone or automatic voice registered system and absence of 2 times consecutively, please make an appointment in person. If automatic voice registered for appointment is full, no spots are reserved for appointment in-person.
Door Closure During the epidemic, please follow the latest announcement of the hospital	Accessible Door Hours (6:00~22:30) Door closure hours (22:30~6:00). Staffs should use their Employee ID badges and family members should use Family card for accompany patients in B and C building to enter and exit. During door closure time, no cars will be allowed to enter 2^nd parking lot; cars are only allowed to exit during this time. All cars are to be parked at 1^st parking lot.
